Examples of the monitoring loop
- Checking whether you understood every word in a conversation.
- Repeatedly replaying audio to feel completely certain you heard it.
- Testing whether your attention has returned to normal.
- Monitoring swallowing, blinking or breathing and fearing you will notice it forever.
- Comparing the present moment with how effortless attention used to feel.
Not every sensory difficulty is OCD
Hearing changes, tinnitus, attention difficulties, anxiety and other conditions can overlap with these experiences. New or persistent hearing or physical symptoms may need medical assessment. An OCD explanation should not replace an appropriate evaluation of another possible cause.
The struggle to stop noticing
When success is defined as never noticing a sensation, checking whether it is still present can keep bringing it back into focus. Assessment explores the feared consequences of awareness and the actions used to control it. The aim is not to turn every ordinary sensation into an exposure exercise.
How therapy approaches it
Where OCD is part of the difficulty, CBT with ERP may help change checking, avoidance and mental control strategies. Work is tailored to your symptoms and can focus on participating in daily activities even when awareness is imperfect. Exercises should be agreed with a clinician rather than used to ignore a new medical concern.
In an enquiry, mention the main difficulty and any relevant assessment you have already had. Detailed exploration can take place during the initial appointment.
